Service Provider Registration Process Overview

NOTE:

  1. This is an step-by-step on-line registration description. The service provider who uses this process should familiar a basic computer operation, has the computer connected with Internet and has a personal email account.

Complete the TBI Project Service Provider Registration Form - Click here to GO there now. TBI Project office sends a received notification after the application has been received. If the application is approved, the coordinator activates the service provider’s account sending the registration kit to apply for a Certification Key. Log into the TBI Project / Health Professional / Registered Service Provider, with the login ID and login password. Do this step 2-3 days after applying for the Certification Key. Go to the same link where you applied for the Certification Key to check on the pending Certificate Key. Note: For security reason, this is a mandatory step.·	The first time you log into, the system, you will be asked to change the default password. Look for your registration kit through email/mail.  Go to the web link to apply for Certification Key. Authorize the Certification Key to the service provider. Coordinator contacts the college or other body to verify registration information. The committee sends the review result to the coordinator. Coordinator receives verified result from the college. Coordinator sends application and verified result to the TBI Project committee.
