The development of an Integrated Rehabilitation System for Traumatic Brain Injury in the Champlain District which manages “The Seams” when clients are transitioning the system, (moving from service to service or from site to site).

The development of a bilingual centralized transition tracking resource system for Traumatic Brain Injury:

• Client transition data
• Tracking of referrals
• Care and Services resource database and directory
• Increase access to TBI information re: care, services and Brain Injury information by providing on-line resources and on-call assistance

Ensure the outcome evaluation of the project measures:

• Efficiency
• Client centeredness
• Functional measures
